Project Snapshot – PEAChY O and M

PEAChY-0: Pharmacological Emergency management of Agitation in Children and Young people – A randomised controlled trial of Oral medication

PEAChY-M: Pharmacological Emergency management of Agitation in Children and Young people – A randomised controlled trial of IntraMuscular medication

Trial Designs:

Randomised, controlled, open-label, multi-centre, phase III superiority effectiveness trials of: ORAL olanzapine vs ORAL diazepam and IM olanzapine vs IM droperidol.

Aim:

To determine in children and adolescents aged ≥9 years to <18 years with acute severe behavioural disturbance (ASBD) whether oral olanzapine is more effective than oral diazepam and whether IM olanzapine is more effective than IM droperidol at achieving successful sedation at 1 hour post randomisation, without the requirement of additional medication. The Sedation Assessment Tool (SAT) will be used to score sedation and agitation levels – a SAT of ≤0 will be considered “successful sedation”.

Setting & Target:

7 PREDICT paediatric ED sites in Australia.  A total of 348 participants will be recruited in each trial.

  • Royal Children’s Hospital Melbourne – HREC amendment approval for PEAChY-O obtained 26th August. Recruitment commenced 18th October, 2021. HREC approval for PEAChY-M obtained 6th September, 2021. Recruitment commenced 20th October, 2021.

  • Perth Children’s Hospital
  • Women’s and Children’s Hospital, Adelaide – RGO approval obtained 2nd November, 2021
  • Children’s Hospital at Westmead
  • Gold Coast University Hospital – RGO approval obtained 25th October, 2021

PEAChY-O Status: Six participants enrolled.
PEAChY-M Status: Two participants enrolled.

Update:
  • Ongoing review of recruitment processes and documents continues at RCH.
  • Education to commence recruitment at participating sites anticipated to commence in February, 2022.

Implementation research meeting – March 2022

PREDICT members who have previously been involved in implementation research studies or are involved in current work, have been invited to attend a one-day online meeting ‘Implementation research in the PREDICT network – what is implementation research, what have we achieved and where are we heading?’ in March 2022.

The aims of the meeting are to:
i) provide education on the basics of implementation research
ii) provide examples of implementation research that have been undertaken in other clinical areas
iii) discuss future implementation research projects and implementation research training needs for the PREDICT network.

PREDICT members meeting 2021

The annual PREDICT members meeting was held online via zoom on October 20th 2021 and attended by 57 members.  The meeting included a range of informative presentations from the project leads, members and the executive team.  A recording of the slide presentations will be available soon and posted in the secure staff area of the website for those who missed the meeting or want a recap of events.

We look forward to another exciting year of collaborative research and another online and/or face-to-face meeting in 2022!

Getting to know you – meet Amy Ting

Our “Getting to Know You” segment ensures PREDICT members are aware of new members, their interests and areas of expertise and where they are located.

This month we introduce Amy Ting from the RCH in Melbourne and Canberra Hospital.  Welcome to PREDICT Amy.

“I am a Paediatric Emergency doctor currently completing my training at the Royal Children’s Hospital, Melbourne. I am also working as an Emergency Physician at the Canberra Hospital Emergency Department. In my PREDICT liaison role, I am looking forward to taking an active role in research and making a contribution to improving paediatric clinical care.”

Project Snapshot – The Kids are Not Ok: Delphi study

The Kids are Not Ok: A Delphi Study to identify consensus research priorities and core outcome sets in paediatric emergency mental health presentations

AIM
  • To determine a prioritised list of research questions and identify a set of core clinical and mental health outcomes that should be researched for children and adolescents below 18 years if age who present to the ED with mental health concerns.
Participants
  • Children/young people under 18 who attended ED with mental health concerns and their parents/carers.
  • Doctors, nurses, mental health clinicians, police, paramedics, schoolteachers, school counsellors, hospital security
Ethics and governance
  • Monash Health HREC approval has been granted for this multi-site project.
  • This project requires site approvals across ALL participating hospitals, AND approvals from other participating organisations prior to the study being able to commence.
  • There have been delays with this project commencing due to various requirements/ application processes from different organisations in the different states.
  • Currently we have governance approvals and fully executed contracts from 9 participating hospital sites. 10 hospital sites are pending governance approvals.
  • We also have approvals from:
    • VIC – Victoria Police, Ambulance Victoria,
    • QLD – Queensland Ambulance Service,
    • WA – Western Australia Police Force, St Johns Ambulance, Department of Education – Western Australia,
    • SA – South Australia Ambulance Service, Department of Education – South Australia,
    • NSW – Department of Education – New South Wales
Recruitment STATUS
  • Recruitment will begin once ALL participating sites have approval. We are aiming to commence participant recruitment in Feb 2022.
Primary outcome
  • To develop a prioritised set of research questions and identify areas, domains and outcomes that should be measured to improve care provided in the Emergency Departments for children and young adults presenting to the Emergency Department with mental health concerns
